If you have a change in your circumstances, you are required to complete a change request form online alongside uploading any relevant documents to evidence your change.
Adding an adult or child to your application
When you want to add someone on to your housing application that you wish to be rehoused with, you will need to upload ID to prove who they are.
Please refer to the allocations policy about who you can include on your application to be rehoused with.
If you are adding on your child/children, you will need to upload a copy of their full birth certificate. If you share custody of a child/children with someone else, we will decide who the main provider of care is by looking at any legal custody agreement in place.
In the absence of a legal custody agreement, we will look at who is paid child benefit or tax credits and whom the child/children stay with for the most nights each week.
If you are not the main care provider, your child/children will not normally be considered as part of your household for the purposes of the Housing Register.
For adding on a partner, you will need to upload two different forms of ID for them. If your partner is not a British national, a valid passport along with proof of their current immigration status/EU settled status should be uploaded.
If your partner is a British national, you must upload two different forms of ID from the list below:
- full birth certificate
- medical card
- marriage certificate
- driving licence
- National Insurance card
- Valid passport
- Freedom pass
Notifying us of a change of address
If you have moved, you will need to upload proof of your new address. If you have a tenancy agreement, licence agreement or lodgers’ agreement for your new address, you will need to upload this, along with at least one other proof of address document.
If you do not have a tenancy, licence or lodgers’ agreement, you can provide us with two other proof of address documents. The following is an example of some of the documents you could provide:
- main bank statement (i.e. the account where wages / benefits are paid into)
- utility bills
- council tax bill
- credit card bill
- GP/Hospital letters
- Phone/internet/TV bills
- itemised postmarked envelopes (not from the council)
- polling cards
- insurance documents
- invoices for online purchases
- a letter from an employer, a government agency, a college or a school confirming the dates that they have known you and the address history they hold for you.
This list is not exhaustive, and we are happy to accept most types of documents if they have your name, address and a date on them. We ask that you provide a variety of documents from different sources, organisations and companies.
Notifying us of a pregnancy
If you are advising us about a pregnancy for you or someone else on your application, you will need to upload evidence of the pregnancy. You can provide one of the following:
- MAT B1 form
- Official medical letter that confirms the expected due date
We will not accept scan pictures.
Notifying us of a separation
If you are legally married and have separated from your spouse, you will need to upload proof of separation, which could be one of the following:
- Decree Absolute or Final Order
- Legal separation document
Where possible, the document must show the date of separation.
Request health or medical priority
If you or someone on your application has a severe long-term limiting illness, or a permanent and substantial disability and their health or quality of life is severely affected by the home you live in, you will need to complete a change request form and upload current medical information.
You can request a printout of your medical records, which is free and easy for you to request from your GP surgery or NHS App.
You can also upload:
- any recent consultant/specialist or hospital discharge letters
- Medical reports from Occupational Therapists, Physiotherapists, Social Workers etc.
- Diagnosis reports (autism, ADHD).
Please do not submit appointment letters or benefit letters. Whilst these letters may evidence your disability, they do not provide specific information on how your housing situation affects your health or ability to access essential facilities.
The above examples of changes in circumstances are just a few examples but there are many others, and you should consider carefully about whether the change that you are reporting, requires evidence to be uploaded and what type of evidence.
If we have not received the relevant evidence or require further information/evidence, we will get in contact with you. However, this can delay the update of your application, so it always best to provide all information and documents as soon as possible to avoid delays.
